Payment Schedule

Four (4) ten week payments as follows:

  1. At the time of your Registration (Classes begin August 4 2025)

  2. Between October 20 and October 25, 2025. Written Withdrawal BEFORE October 6, 2025

  3. Between January 12 and January 17, 2026. Written Withdrawal BEFORE December 29, 2025

  4. Between March 30 and April 4, 2026. Written Withdrawal BEFORE March 16, 2026

By submitting this form, you are agreeing to our terms for a ONE year commitment and will be responsible for ALL associated fees. This is on a YEARLY BASIS (40 Weeks), from August 5, 2025 (or your beginning date) through June 2025 Recital. Fees will be prorated for students admitted after the start date.

YOU are responsible for paying tuition on the above dates. Invitation to Dance does not issue billing statements; however, a REMINDER will be posted in the waiting room. If it becomes necessary to send delinquent reminders, including late fees, an administrative charge will be added. To WITHDRAW from the program, a WRITTEN NOTICE or email (myitdstudio@gmail.com) must be received BEFORE the withdrawal date to not be charged for the next pay period. A $5 late fee will be charged for EACH WEEK passed the above due dates. No exceptions!

STATEMENT OF INTENT/DISCLAIMER

I hereby consent to my child's participation in the programs offered by Invitation to Dance and AGREE to pay for the entire program year, August 4, 2025 (or my beginning date) through June Recital 2026). I may CANCEL this agreement and WITHDRAW my child by PROVIDING a written notice FOURTEEN (14) BUSINESS DAYS PRIOR to the First Day of the installment payment week, as noted on page 1 of this contract. Withdrawal notice must be sent via certified/registered mail to Invitation To Dance, 108 East Stansifer Avenue, Clarksville, IN 47129 OR email to myitdstudio@gmail.com. l am responsible to receive confirmation from ITD that my email was received before the Withdrawal deadline. I understand that Hawley Hughes and the Instructors are NOT responsible to relay/deliver my written notice. I AGREE to pay the ENTIRE TUITION PAYMENT OPTION plus any late fees in which my child had any participation or was scheduled to participate, regardless of the date of termination. I agree to pay cost of collection, including attorney fees. I am aware that there are risks of physical injury associated in participating in said programs and activities. I hereby waive, on behalf of my child and myself, our personal representatives, successors or assigns, any claims for damages that we may have against Invitation to Dance, Hawley Hughes, faculty, chaperones and agents, whether hired or volunteer, for any injury sustained by my child or loss of property during attendance while participating in any programs or related functions conducted by ITD (Including transportation). In case of emergency, I authorize Invitation to Dance to call a doctor or emergency medical service and I agree to assume every financial obligation of the emergency
